Abyssal Wretch
Terminally mutated humanoid. An aberration.
Imagine a humanoid so mutated and corrupted that it looks like it’s constantly melting. It has no feelings, only maddening pain.
Feral from pain, disgustingly resilient, hates life.

It resists mind attacks (because it’s an aberration). It can’t die unless it’s would-be killer saves vs fear. Also, its constant mutating allows it to heal all its hit points at the beginning of its turn if it is not dead.

Bite and Claws. The wretch makes a melee attack (1D4).
Abyssal Hulk
Walking exploding tumor. A large aberration.
Wretch that mutated so much it has become a hulk of regenerating flesh on wobbly legs.
HD: 1 +15HP | Armor: none |
Hit it: easy | Dodge it: normal |
Move: slow |
It resists mind attacks (because it’s an aberration). It can’t die unless it’s would-be killer saves vs fear. Also, its constant mutating allows it to heal all its hit points at the beginning of its turn if it is not dead.
Attacks (1/round)
Explode: The hulk falls prone. At the beginning of its next turn, it dies, inflicting (3D8) damage to all creatures and objects nearby, or half to those who manage to dodge.
Random Encounter
- Monster: 2D6 abyssal wretches & 0-1 hulk.
- Lair: A pentagram with flesh tumors surrounding it and incantations to summon a sibriex.
OR
Omen: A pained shriek ending in a gargle and rattling noises, close-by. - Spoor: Walls tainted with freshly grown flesh tumors.
- Tracks: Distant moans, shrieks and gargles.
- Trace: Writings from a person obsessed with a dark summoning.
- Trace: An aggressive, mutated vermin.

Your studies of the aberration have changed you in horrible, gruesome ways: tumors constantly grow and fall off …
- … your leg. You might be able to hide this. -1 movement.
- … your arm.
- … your hand, which fuses with an item of your choice.
- … your chest. Gain 1D4 temporary hit points every time you rest.
- … your face and it blocks one of your eyes.
- Reroll. You know the spell word Flesh and gain one Spell Die.

Abyssal wretches from Mordenkainen’s Tome of Foes bring to DnD the trope of the demon-infected mutant-zombies which have been popular in Sci-Fi video games since at least Doom. I wanted to make them different from the undead zombies by giving them extreme regeneration. I added the hulk variant as an homage to my favorite spin on the mutant-zombie trope: Halo’s Flood Carrier. — SaltyGoo
